Do you require a replacement CAPLA name badge?

 

Step 1:  Scroll down to "Items for Purchase" to order a name badge by entering a 1 in the Qty box and then clicking 'Register for these items' to proceed.

 

Step 2:  You will then need to confirm the spelling of your name and another option by selecting the 'Item Options' link when prompted. 

 

Step 3:  Select the "Method of Payment" as "Cheque" and then click "Proceed >"

Step 4:  Review & Make Changes if necessary and leave comment in comments box if you wish

Step 5:  Confirm Registration

 

The next Badges will be manufactured in April 2012.  Payment will not be requested until then.  New Members are not required to order a badge as new members from Nov 1, 2011 forward will automatically receive their badges in April 2012 or sooner.
